Hurwitz, Assistant Director Reentry Services Division SUBJECT: Psychological Reconstruction Inmate Epstein, Jeffrey (76318-054) Inmate Jeffrey Epstein (76318-054) died by suicide on August 10, 2019, while housed at the Metropolitan Correctional Center (MCC) in New York. The attached psychological reconstruction was completed by Drs. [redacted] National Suicide Prevention Coordinator, [redacted] Sex Offender Treatment Programs Coordinator, [redacted] Mental Health Treatment Coordinator and Mr. [redacted] Correctional Services Administrator, Northeast Regional Office. A summary review of these and other recent reconstruction findings is forthcoming. Distribution of this report is limited to staff named in this memorandum. Recommendations at the conclusion of the report should prove beneficial to staff at the facility and will be used to inform our national suicide prevention program. Within sixty days of receipt of this memorandum and report, please provide me with a written response which outlines corrective actions as well as a plan for implementation based on recommendations contained within.
